As specified below, the following information should appear in compatible, legible type at the bottom of the back cover or last page of printed pieces, at the bottom of home pages and at the end of the last page of electronic documents.

- Address
- Phone Numbers
- Fax Numbers
- Contact e-mail address(es)
- Kellogg or departmental/targeted URL
A note about URLs. Keep them as succinct as possible, using only the elements required to arrive at the intended Web location. Avoid extras such as “http://www.” or “.aspx” or other suffixes, including a final “/” if they are not essential to proper functioning. To make sure, always test a URL before publishing it for others to use.
A copyright notice, as shown below, should appear on all printed communications.

A tracking code, in small type, should be placed on the inside back cover (or equivalent page) of printed pieces. The code includes the date, quantity (for printed pieces) and who produced the piece. For example, the code 8-10/15m/KSM-CF means the piece was published in August 2010, 15,000 pieces were printed and it was produced at Kellogg School of Management by Carol Forsythe.
